foot:backward=* and its counterpart foot:forward=* are used by some mappers to indicate a feature can only be used in one direction by pedestrians.

In the past, some mappers have used oneway=yes for this purpose, but it goes against the definition of the oneway-tag which applies only to vehicles. Separate tag oneway:foot=yes was thus invented which does not suffer from that ambiguity, and while significantly more widespread than foot:backward=no, proponents of this tag dislike it (for a reasoning that a subtag should not contradict the main tag).

Some mappers seem to have thus resorted to mapping multiple of those tags in combination, presumably in attempt to make everyone happy.
